The Problem

Directors of Care and Administrators at long-term care homes rely on care teams to write progress notes, behaviour notes, shift notes, assessments and incident reports, but this information is scattered across notes, forms and shifts in the EHR. Today, understanding a resident's health picture or spotting a pattern (like falls clustering overnight or hydration drops preceding a fall) requires manually reading through notes and rebuilding spreadsheets. This manual process costs hours of staff time, leads to missing documentation and non-compliance gaps that surface only at inspection, and can delay care decisions that reimbursement and resident safety depend on.

The Solution

Ascenix is a read-only incident intelligence layer that connects to a care home's existing EHR and other systems via API, without changing how staff chart or writing anything back to the record. It aggregates notes, assessments and logs into automated incident reports, flags missing documentation and compliance gaps, and produces a Risk Radar that ranks every resident by incident frequency and trend across modules like falls, dehydration, behaviours, wounds, infections and medication errors. An AI assistant (Glenn) can answer specific questions across modules, for example correlating hydration intake with fall timing for a single resident, and the product also generates automated 24 hour shift reports and home-by-home incident comparisons for leadership.
